ECG Challenge: Palpitations and diarrhea in a physical examination of the young woman, including vitals, is normal even if the patient is tremulous. Blood sent for labs, so look for clues at the ECG. History: A 39-year-old woman presents the emergency department complaining of palpitations. She says she has also had "a little diarrhea" today, but she has similar symptoms at any time she is stressed. She denies any fever, abdominal pain, chest pain, sincope or near syncope. It also denies any relevant medical history but says it takes furosemide as necessary when your legs swell. He claims that his regular doctor ordered a full blood panel and an echocardiogram to look for a cause of inflammation and that everything was fine. Test: Your exam is essentially normal including your vital signs. There's no root or peripheral edema. However, it seems a bit agitated/tremulous. Replies1. What are the ECG findings? QTc long 550 ms, as well as tachycardia at a speed of about 125 bats/min. It is difficult to know if the rhythm is sinusal or not.2. DiscussionHypokalemia is often asymptomatic but can cause widespread weakness, constipation, and polyuria, as well as heart dyrhythmias that in turn lead to palpitations or almost syncope. When potassium levels drop below 2 mEq/L, muscle paralysis may occur. Changes in the ECG may occur before overdose symptoms and usually include flat or inverted T wave, ST depression, U-waves and prolongation of QT, PR and QRS intervals in more serious cases. Atrial and ventricular dyrhythmias may also occur. There are many types of hypokalemia and can be classified into alkaloid and non-alkaloid causes (see the featured part of the page taken in Figure 2 to the right). If you look at the list in the Figure you will notice that diarrhea causes non-alkatic hypocalemia, but diuretics cause alkalotic hypokalemia. Since the patient's bicarbonate level was extremely high (48 mEq/L), the furosemide is the most likely culprit in this case. Treatment of hypokalemia naturally begins with potassium management: oral replacement is generally considered safer, with IV supplement reserved for unstable patients or those with levels below 2.5 mEq/L. In cases where hypokalemia is caused by cell change, be careful not to be overly treated, as hyperkalemia bounces is a risk when change is reversed. It is also, of course, important to reverse, remove or treat any causative factor, such as medication in this case (for more information, refer to the highlighted part of the page photo in Figure 2 above).
